EZI DEBIT NEW CUSTOMER FORM

Please note we are now using EziDebit to process our various payment plans. This gives members the options to now use a credit/debit card to pay their subscriptions monthly, quarterly or half yearly. Members wishing to use this option will need to fill out an Ezidebit authorisation form.
